ABOUT SAFETY IN MOVEMENT CLASSES with Dana Hercbergs on Zoom

Thank you for your interest in my movement classes. I aim to accommodate people's physical needs and to offer modifications for a safe and satisfying experience in class. The best way to support your needs and safety is to notice your body's needs and limitations, and to either modify or abstain from a movement or posture that could lead to injury. If you're not sure, asking me is a good way to check.

Because I may not know participants' medical conditions or limitations in a given class, I will ask you to sign the following standard liability waiver prior to the class:

 

WAIVER AND RELEASE OF LIABILITY AND INDEMNITY AGREEMENT

It is recommended that you seek the advice of a physician before beginning any new exercise routine.

Upon signing this Agreement and thereafter, you agree that if you participate in or attend any movement classes with Dana Hercbergs, you are doing so at your own risk and assume the risk of any and all injury and/or damage you might sustain. Your assumption of risk includes but is not limited to the use of any equipment (props). You agree that you are voluntarily participating in the aforementioned activities and assume all risk, known and unknown, associated with same. This release is not intended as an attempted release of claims of gross negligence or intentional acts.

YOU ACKNOWLEDGE THAT YOU HAVE CAREFULLY READ THIS WAIVER AND RELEASE AND FULLY UNDERSTAND THAT IT IS A RELEASE OF LIABILITY, AND EXPRESS ASSUMPTION OF RISK AND INDEMNITY AGREEMENT.
